AP Knee Resection: How to perform

Description

Deformity correction and the restoration of the anatomic alignment should be achieved to maximize the longevity of the replaced components. The femoral component is chosen individually for each person according to the amount of healthy bone present. 

This procedure allows performing the total knee arthroplasty (TKA) in a limited AP view of the knee when the femoral is hidden. TKA is the replacement of the knee joint with tibial, patellar, and femoral components. 

The AP knee resection allows estimating the distal femoral cut and proximal tibial cut perpendicular to the mechanical axis and the tibiofemoral angle. The mechanical axis is estimated, typically 6° to the anatomical axis.


Measurements

apkr_1The measurements made by the procedure are displayed in the image: 

  • Medial Femoral Cut Length (mFCL): a medial length in the femur to perform the cut;
  • Lateral Femoral Cut Length (lFCL):  a lateral length in the femur to perform the cut;
  • Medial Tibial Cut Length (mTCL): a medial length in the tibia to perform the cut;
  • Lateral Tibial Cut Length (lTCL): a lateral length in the tibia to perform the cut;
  • Anatomical Tibiofemoral Angle (aTFA): the angle between the femoral and tibial anatomical axis.

To show this procedure, PeekMed® needs additional references, such as:

  • Anatomical Axis of the Femur (AAF): line from the center of the proximal femur to the center of the distal femur;
  • Anatomical Axis of the Tibia (AAT): line from the center of the proximal tibia to the center of the distal tibia.

How to perform

After selecting this procedure in the sidebar, as well as, the side where you will perform the procedure, you must mark the points to conclude it. To see the caption of each point you need to click on this button .

Beware: you can change the position of the points at this stage or later. To do this, simply click on each handle with the left mouse button and move them to the most suitable position.

This procedure cannot be performed simultaneously with Total Knee Arthroplasty Resection as the latter contains all of AP Knee Resection points.

Advanced Configuration 

After positioning the resection handles in the extremities of the femur and the tibia, it is possible to perform auto templating of the femoral and tibial components using this option on Object edition. 

After clicking on this button, the auto templating feature automatically detects the templates that better match the femoral and tibial components’ width and place them in the planning view, with the expected correct alignment.

By clicking Finish, PeekMed® will automatically add the selected templates to the plan. You can change the selected templates and their configuration in the Objects section, clicking on the template’s configuration icon. It is possible to change the templates via the “Update List” buttons.
